Description:Jefferson County Public Health is currently hiring a
Nurse Home Visitor for its Nurse-Family Partnership (NFP) Program. This position provides you the opportunity to work for a county that thrives on working collaboratively and overcoming challenges. This position in Jefferson County Public Health (JCPH) involves community-based public health nursing work to promote and protect the health of birthing individuals, families, and populations using knowledge from nursing, social, and public health sciences. The nurse home visitor is responsible for maintaining the highest standards in clinical nursing practice and adherence to the Nurse-Family Partnership model, policies, procedures. The Nurse Home Visitor works as part of a team to deliver high-quality, preventive health services.
SCHEDULE: Please note this position has a flexible schedule with a 4-day work week. This position is
hybrid, with remote, in-office and on-site work in client homes.
COMPENSATION: Hiring Range: $78,234.13 - $86,926.81 USD Annual
Compensation will be determined based on education, experience, and skills.

ESSENTIAL DUTIES:Involves professional nursing work and case management in a home visitation health care setting to include but are not limited to:- Adheres to the nursing process and the Nurse-Family Partnership model of home visitation.
- Develops therapeutic relationships with birthing and parenting individuals and their families in a home visiting environment.
- Assesses physical, emotional, social, and environmental strengths and risks of birthing and parenting individuals and their families.
- Provides education, support, and referral resources in assisting birthing and parenting individuals and their families in attaining their targeted goals
- Works as part of a nursing team to deliver care in a culturally appropriate and client-centered manner to diverse populations.
- Works with clients using Motivational Interviewing, Trauma Informed Care and a Strengths Based approach to support positive behavioral changes during pregnancy and beyond.
- Conducts care coordination and health management with clients.
- Documents and communicates client care through maintaining accurate, complete, and timely electronic and paper medical records while maintaining confidentiality.
- Represents the Nurse-Family Partnership and JCPH vision, mission, and model in actions as well as verbally to both internal and external customers and colleagues.
- Maintains clear, effective, open, and honest communication with both internal and external customers and colleagues.
- Participate in NFP and community outreach events as requested.
- Precept and/or mentor nursing students and new employees.
- Participate in program planning and evaluation, quality improvement processes, and policy and procedure development.
- Participate in ongoing relevant professional education.
- Represent JCPH and the public health nursing discipline on internal and external committees and workgroups to promote its concepts including health equity and social determinants of health.
- Other duties as assigned.
QUALIFICATIONS: Minimum Qualifications:
- Bachelor's degree in nursing and minimum one(1)year of work-related experienceoran equivalent combination of education and experience. For example, applicants will meet minimal qualifications if they have anAssociate’sdegree and 3 years of experience.
- Licensed as a Registered Nurse (RN) in Colorado, or in a state that has enacted the RN Nurse Licensure Compact at time of employment.
- Current CPR Certification at hire.
Please note that preferential consideration for interviews will be given to those candidates that have maternal child health, public health, community health, home visiting, home health and/or mental health experience. Preferred Knowledge, Skills and Abilities:
- Two (2)additionalyears of experience above theminimumrequirements.
- Experience in use of Electronic Health Records.
- Spanish speaking abilitypreferred butnotrequired. JCPH has a policy to additionally compensate workforce members whoare able todemonstratebilingual language skills.
- Experience working with Nurse-Family Partnership/Nurse Home Visitation Programs.
- CLC or IBCLC certification and/or work with breastfeeding mothers.
- Experience working with teenagers.
