Description

The City of Santa of Santa Fe Springs Police Services Department is seeking a highly-motivated individual for the position of Social Services Case Manager.

Police Services Department (Homeless Engagement): The Social Services Case Manager plays a vital role in assisting individuals experiencing homelessness and their families in navigating and accessing community services. This position requires a flexible, self-motivated, and proactive professional with strong interpersonal skills. The ideal candidate will have experience working with unhoused individuals, possess knowledge of street culture, and demonstrate the ability to build trusting, supportive, and empathetic relationships with clients.

Employment Type: Full-time position represented by the City of Santa Fe Springs City Employees Association.

Work Schedule: Social Services Case Manager is assigned a 4/10 work schedule between the hours 7:30am-5:30pm. Must be able to work a flexible schedule to accommodate City needs that may include evenings and weekends.

POSITION PURPOSE:
Under general supervision, delivers comprehensive social services to individuals and families in crisis, with a focus on those experiencing homelessness. Responsibilities include but are not limited to intake, assessment, meeting with clients, providing supportive and referral services, crisis intervention, monitoring client progress/outcomes, data entry/collection, record keeping, reporting and managing long-term care plans. The Case Manager serves as a vital link between law enforcement and community-based care, ensuring clients receive appropriate and timely support. The Case Manager will work closely with the Housing Navigator.

Receives general supervision from the Homeless Services Supervisor in the performance of assigned duties.

EXAMPLES OF DUTIES AND RESPONSIBILITIES:

A. Held in Common:

B. Essential Job Specific Duties:
Conducts client interviews and assessments. Provides referrals to specialized social services or public assistance programs. Determines and documents eligibility for social services benefits in accordance with applicable regulations and division policies. Assesses client’s needs, develops individualized care plans and connects clients with appropriate resources. Responds to requests for homeless services, including emergency food and shelter assistance. Educates clients about available resources, benefits and entitlements. Offers referrals to local agencies for specialized services for mental health, substance abuse, domestic violence, and other specialized community services, to improve quality of life. Assists clients with completing forms, translating and interpreting documents, writing letters, and obtaining required identification (i.e. birth certificate, income verification, etc.) or benefits. Build and maintain strong partnerships with community and non-profit organizations and service providers. Advocates on behalf of clients. Serves as a liaison between clients and service providers. Conducts outreach visits to encampments and other locations as needed to assess client needs. Monitors and distributes donations and in-kind goods, such as food, hygiene kits, clothing, and bedding to clients as needed in coordination with Gus Velasco Neighborhood Center. Engages local businesses, organizations, and community members to secure donations and support services. Maintain accurate and confidential case records in the automated Case Management System and assist with data collection. Evaluates client’s social functioning and related data to develop and adjust social case plans. Reports suspected elder abuse or neglect to Adult Protective Services (APS) as required by law. Assist police personnel in non-enforcement situations involving homeless clients and obtain appropriate care for clients. May assist safety personnel with non-psychiatric emergencies such as transporting clients to substance abuse treatment. May interface with hospital personnel regarding emergency services and recuperative care for homeless. Participate in meetings and conducts site visits with representatives of contract or service providers. Interact appropriately with staff, management, health and welfare agencies, non-profit/community-based organizations, public and private representatives, the public including persons experiencing homelessness, housing groups, committees, other public agencies and others in the course of work.
C. Other Job Specific Duties:
Administers and disburses special program funds as directed.Maintains direct communication and working relationships with agencies providing supportive services.Maintains accurate case records and prepares related reports as required.Track and manage monthly budget expenditures related to case management services and special programs.Purchases and manages program supplies and materials.Performs other related duties as assigned.
