Description
Job Location: This position will be located at 930 Wildwood Drive, Jefferson City, MO 65109 - In office with a possibility for telecommuting.
Why you’ll love this position:A community health worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. In this exciting and fast-paced position, you will coordinate, promote and support statewide CHW initiatives to improve health outcomes for all Missourians. You'll work with some of the most talented, passionate and inspiring health care workers in the field to connect them to resources, education and funding.
- Enhance and maintain a partnership network to support the growing CHW workforce across the state of Missouri.
- Coordinate and provide educational opportunities for CHWs on the approved credentialed curriculum, chronic disease prevention and management and other identified training topics
- Provide support to the Community Health Worker Association of Missouri (CHWAM) and the Statewide CHW Advisory Committee to provide recommendations on effective and efficient utilization of CHWs in improving health outcomes for Missourians.
- Build the infrastructure necessary to increase the number of credentialed CHWs in the state and promote CHW workforce sustainability.
- Create, facilitate, and monitor letters of agreement and contracts with partners to achieve program goals.
- Contribute to required grant reports and updates.
- Promote sustainability of the CHW program across the state
Minimum Qualifications: - A four year degree in a related field is preferred.
- Excellent relationship-building skills. Strong time management and organizational skills.
- Program management experience
- Experience writing documents such as grants, contracts, letters of agreement, and other correspondences.
- Lack of post-secondary education will not be used as the sole basis denying consideration to any applicant.
