The COVID-19 pandemic has intensified mental health challenges, especially among youth in Jefferson County, with diverse youth populations reporting increased isolation, mental health struggles, and hesitancy in seeking help for nicotine cessation. JCPH aims to offer youth-centered, culturally sensitive cessation services and empower young people to be key decision-makers in tobacco prevention efforts.

Jefferson County Public Health (JCPH) is seeking a dedicated Public Health Nurse, Program Lead to build and administer a youth-driven nicotine cessation program to address increasing rates of youth vaping and nicotine dependence. This initiative focuses on integrating youth-led strategies to prevent vaping and offer accessible cessation resources for adolescents and young adults aged 12-21 . This role involves building partnerships with youth and community partners to develop and implement youth-led outreach, advancing evidence-based nicotine cessation efforts in clinic, and evaluating the effectiveness of these interventions.

This position involves clinic and community setting public health nursing work to promote and protect the health of individuals, families and populations using knowledge from nursing, social, and public health sciences. This position is located within the JCPH Clinic Services division, which provides immunization, sexual health, and harm reduction services to our community. This role will also partner extensively with the JCPH tobacco prevention team.

The ideal candidate is passionate about health equity and will excel in a clinical nursing setting, specifically in a public health role. This individual will be highly motivated, self-directed, and work creatively. They are adept at developing relationships and trust with youth and will be able to effectively engage with internal and external partners.

Schedule:

This position typically operates on a 4-day work week Monday-Thursday with occasional Friday, evening and weekend shifts as needed. This position is anticipated to allow for a hybrid work schedule; however, in-person work in clinic and the community will be required .

This position will be required to travel within the county for outreach projects.

Compensation:

Hiring Range: $80,000-$95,000 USD Annual

Compensation will be determined based on education, experience, and skills. Please note that this position is grant-funded, and funding is not guaranteed beyond June 2026 .

Essential Duties:

The Public Health Nurse Lead - Youth Nicotine Cessation Program will have primary responsibility for developing and administering JCPH’s youth-centered nicotine cessation services, to include:

1. Youth Engagement & Partnerships:

• Collaborate with a diverse Youth Advisory Board (YAB) focused on youth-led initiatives for vaping prevention and nicotine cessation.
  

• Facilitate outreach efforts and gather youth input at schools, community events, and other youth-focused spaces to promote services and incorporate youth feedback into program design.
  

2. Vaping Prevention & Cessation Programming:

• Develop policies, procedures, protocols, and standing orders for the implementation of youth-centered nicotine cessation services in clinic, including Nicotine Replacement Therapy (NRT), coaching, and behavioral support interventions.
  

• Implement a multi-channel approach to cessation support, including telehealth, text/call follow-ups, peer support, and clinic visits.
  

• Participate in ongoing collaboration with the Chief Medical Officer (CMO) team and Public Health Nurses to deliver and measure the effectiveness of NRT and ni cotine dependence services delivered through JCPH clinics.
  

3. Collaboration & Community Outreach:

• Work closely with community partners and key stakeholders to provide accessible, comprehensive, evidence-based nicotine cessation interventions for youth.
  

• Work with JCPH’s Health Communications team to develop and advance youth-led outreach campaigns that reach diverse communities with accurate , engaging information on vaping and cessation.
  

• Leverage existing partnerships to promote cessation services, build referral networks, and gather input from diverse youth populations
  

4. Program Evaluation & Continuous Improvement:

• Track key performance indicators, including community engagement metrics, cessation success rates, and youth satisfaction through surveys and feedback tools.
  

• Use Quality Improvement (QI) protocols and feedback loops to continuously refine outreach strategies, cessation services, and youth engagement practices.
  

• Collaborate with research partners to produce quarterly and final evaluation reports as required by grant.
  

5. Sustainability & Funding:

• Community Outreach: Work with schools, mental health services, and community organizations to engage youth in nicotine cessation initiatives.
  

• Program Implementation: Organize outreach activities, including community events and social media campaigns, in collaboration with youth advisors. Utilize the JCPH mobile van to reach youth who may have barriers accessing the traditional clinic environment.
  

• Evidence-based Interventions: Lead JCPH’s Clinic Services Division in developing and implementing youth-specific nicotine cessation interventions with additional training and technical assistance provided by the University of Colorado School of Medicine’s Behavioral Health and Wellness Program.
  

• Data Collection & Reporting: Use surveys and data tools to track engagement and measure program success, adjusting strategies based on continuous youth feedback. Assess community needs. Collaborate with community agencies, boards, and coalitions to address identified needs.
  

In addition to the duties outline above, the Public Health Nurse Lead will be expected to assist in clinic, providing direct client care on-site and in the community as needed and assigned, including the following:

• Improve the health and well-being of individuals, families and the greater Jefferson County community by attending to the Social Determinants of Health, providing high-quality education, and supporting connection to resources and referrals as needed and requested .
  

• Deliver individualized, culturally competent, and ethical client care to individuals and families. Deliver person-centered care based on positive, respectful relationships utilizing reflective communication skills to a diverse population.
  

• Document client care via electronic health records and paper files. Collect appropriate client data, assuring quality and accuracy of the data. Perform routine administrative tasks incidental to nursing functions.
  

• Review client immunization records for compliance and counsel clients about vaccine and medication reactions and effectiveness. Prepare and administer immunizations and medications to clients in accordance with JCPH policies, procedures, and protocols and medical director’s standing orders.
  

• Maintain working knowledge and expertise about immunizations and serve as a subject matter expert on vaccines for the community.
  

• Respond to health care emergencies as needed. Participate in JCPH emergency preparedness trainings as appropriate . Perform duties as assigned during a natural or bio-terrorism emergency.
  

• Promote department programs to community and maintain awareness of current community health care needs.
  

• Precept and mentor nursing students during community/public health rotations. Serve as a preceptor and mentor for new employees.

Minim um Qualifications:

• Bachelor’s degree in nursing and minimum one years of work-related experience in nursing, community outreach, health education, or youth services or an equivalent combination of education and experience.
  

• Licensed as a Registered Nurse (RN) in Colorado, or in a state that has enacted the RN Nurse Licensure Compact at time of employment and;
  

• Current CPR certification at hire.
  

Preferred Knowledge, Skills, and Abilities:

• Strong communication and partnership-building skills, with a focus on youth engagement.
  

• Experience with project management, youth development models, and health equity initiatives.
  

• Knowledge of nicotine cessation programs and evidence-based public health interventions.
  

• Experience working in collaborative environments, facilitating youth advisory boards, or peer-led initiatives.
  

• Strongly prefer experience in public health nursing.
  

• Experience in use of Electronic Health Records.
  

• Spanish speaking ability preferred but not required . JCPH has a policy to additionally compensate workforce members who are able to demonstrate bilingual language abilities.
