The County of Colusa Department of Health and Human Services is seeking a compassionate and dedicated Case Manager to join our team and make a meaningful difference in the lives of children with special health care needs and their families.

As a Case Manager in the California Children’s Services (CCS) program, you will work directly with children and young adults with serious chronic or physically disabling medical conditions and their families. You will assess client needs, coordinate services, provide support and advocacy, and connect families with appropriate medical, community, and social service resources.

The Colusa County Department of Health and Human Services is the largest department in the County, with more than 80 staff members. Our agency serves Colusa County residents through a variety of programs, including CalWORKs, CalFresh, Medi-Cal, In-Home Supportive Services (IHSS), Adult and Children’s Services, Public Health, and Housing. There is currently one position available within the Health & Human Services Department. This recruitment will be used to fill the current vacancy, with an anticipated start date between mid-October and November 1st . This timing will also provide the selected candidate with the necessary program-specific training. An eligibility list will be established from this recruitment to support the filling of future vacancies without requiring the County to conduct a new recruitment.
What's this Job All About?

As a Case Manager, you’ll be the person helping families navigate the CCS program and access specialized care for their children. You’ll coordinate with families, healthcare providers, and community partners to arrange medical services, equipment, therapy, and other supports based on each child’s needs.

The role involves assessing needs, coordinating care, communicating with multiple providers, and helping families understand and access available resources. It’s a great fit for someone who enjoys building relationships, staying organized, solving problems, and advocating for children and their families.

You’ll be part of a collaborative department where you’ll have the opportunity to develop your skills while contributing to services that directly support children with significant health care needs.

Supervision & Examples of Duties

Receives general supervision from the assigned supervisor.

Qualifications

Experience and Training Guidelines -any combination of experience and training that would likely provide the required knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be:

Experience:

Four years’ experience working with the client population in either mental health or social services.

Training:

Equivalent to the completion of the twelfth grade supplemented by specialized training in mental health or social services.
License or Certificate:
Possession of, or ability to obtain, a valid driver’s license.

Supplemental Information

The salary range above consists of 15 steps. Steps 1-6 are annual merit steps with approximately 5% difference between each step, based on successful performance. Steps 7-15 are additional merit steps, requiring 2 years of satisfactory performance at the previous step. Steps 7-14 provide 2.5% increases, while Step 15 offers a 5% increase. The final salary offered will depend on the applicant's experience and qualifications, ranging from steps 1 to 15.
