Description

The Position

The current vacancy is a part-time position (40 hours bi-weekly) located in Ukiah.

The list developed from this recruitment will be used for a limited period to fill open and promotional, regular full-time, and part-time positions in a variety of locations throughout the County, should they occur. Pursuant to current bargaining unit agreements, employees regularly assigned to the coastal region or in Covelo are eligible to receive a 5% assignment premium .

Under general supervision, performs moderate to complex support functions in Health Services. Serves as a vital link between communities and health services to promote health among individuals, families, and groups who lack access to adequate care; determines client financial or medical eligibility for related programs; and may provide childcare service. Relieves professional staff of the more routine duties; offers transportation assistance, performs ongoing assessment and case management, processes documents, manages records, and assists with educational materials. Can serve as a bilingual interpreter. Certain assignments will require immediate supervision by a licensed professional.

Job Requirements and Minimum Qualifications

Duties may include but are not limited to the following:

• Provide assistance and support services to clients and licensed/registered health services staff.
• Assist with case management and coordinate services for clients including referrals to necessary medical, education, social, preventative, and vocational rehabilitation services.
• Assist in developing immediate solutions to emergency problems and expediting delivery of needed services, may conduct very limited basic health screening and assessments such as weighing, measuring, and taking vital signs.
• Make appointments, keep records, gather data, write reports, and interview clients
• Act as a liaison between staff and families by helping families set up appointments for a variety of social and health services and providing interpretation services between families and staff; call to continue to follow-up plans recommended by staff.
• Process and complete assessment and intake forms; determines initial eligibility for program(s); and continue to determine eligibility as required.
• Arrange or coordinate transportation for clients to and from appointments or programs. May include supporting clients at provider or community services appointments.
• Assist with preparation of group meeting, including gathering and/or preparing documents, agendas, reading material and appropriate audio-visual materials.
• Provide childcare, including providing a stimulating environment for children while parents are in group meetings; planning, budgeting, and preparing nutritional meals/snacks; noting any developmental needs a child may have; discussing with parents and client’s counselor; ensuring files are updated with immunization records, etc.
• Answer incoming calls and greet visitors and the public; provide customer services and support; respond to inquiries and/or directs individuals to the appropriate area or assist them with information, providing applications, or answering questions.
• Track and report grant expenditures and deliverables, ensure compliance with grant requirements.
• Work with counselors and treatments providers, assist clients in finding treatment programs; provide tracking and monitoring of treatment services.
• Attend appropriate staff and in-service meetings; attend approved training sessions and job-related education courses.
• Conduct regular, relationship-based home visits with the assigned program recipients. Populations may vary such as:
• Maternal Child Adolescent Health (MCAH): Expectant parents and families of young children to support healthy child development, strengthen parent-child bonding, and promote family well-being.
• Behavioral Health and Recovery Services (BHRS) Mobile Teams: Individual that are homeless with behavioral health conditions to support engagement in services for behavioral health care, connection to housing supports, and other pro health and prosocial activities
• Use a strength-based and trauma-informed approach to assess family needs, develop individualized service plans, and provide parent education.
• Monitor child development and family progress over time using valid tools; support early identification and reporting of potential concerns and connect families to appropriate resources.
• Support families in accessing and navigating medical, behavioral health, educational, and community-based resources through warm hand-offs, visits at provider offices and community locations, and referrals.
• Maintain accurate, timely documentation in accordance with program requirements and standards, including service notes, assessments, and data documentation and reporting. Update client demographic/financial information as necessary.
• Provide support with new staff, including conducting training in home and community settings.
• Maintain a fully active client case load, that is minimally to moderately complex.
• Assist in dedicated programs where program specific requirements will be clearly outlined for example Women, Infant, Children’s (WIC) or California Children Services (CCS) or Communicable Disease (CD) or Maternal Child Adolescent Health (MCAH), Behavioral Health and Recovery Services (BHRS) Mobile Services, BHRS Wellness Centers, and other programs that engage in the community with individuals/families to support connection to a variety of services.
• May act as an interpreter and translator when qualified to do so in contacts involving non-English speaking clients.
• May serve as backup for Integrated Services Coordinator I.
• Perform other related duties as assigned.

MINIMUM QUALIFICATIONS REQUIRED

Education and Experience:

An associate’s degree related to health or human services, child development, social or behavioral sciences or a related field and one (1) year performing related functions in community services, client advocacy, outreach work, eligibility assessment, and /or front office duties in a health care setting.

OR

One (1) year of successful full-time experience as an Integrated Services Coordinator I with Mendocino County.

Licenses and Certifications:

Valid Driver’s License

When assigned to WIC:

Successful completion of both WIC Nutrition Assistant (WNA) and Certified Lactation Counselor certifications within one (1) year of appointment.

Knowledge, Skills, and Abilities

Knowledge of:

• Applicable state, federal and local ordinances, laws, rules, and regulations, including mandated reporting.
• Community based resources including behavioral health, housing, medical, educational support, and other services.
• Needs, problems, and barriers to health care for low-income and minority populations.
• Behavioral health systems, social services programs, eligibility criteria, and referral pathways.
• Process of application for Medi-Cal and other health care coverage.
• Effects of tobacco, alcohol, and other drugs.
• Trauma-informed care.
• Basic screening techniques for tobacco, alcohol, and other drug use and basic techniques for tobacco cessation intervention.
• Principles and best practices of integrated care, cross-system coordination, and multi-disciplinary service delivery.
• Office procedures include filing, record keeping, and scheduling appointments.
• Computer applications and hardware related to the performance of the essential function of the job.
• Basic knowledge of early child development.
• Confidentially practices and professional boundaries.
• Cultural sensitivity/responsiveness and effective engagement with diverse populations.

Skill in:

• Organizing work, setting priorities, and following up on assignments with a minimum of direction.
• Conducting assessments and developing individualized service plans.
• Performing integrated service delivery, case planning, and care coordination.
• Using tact, discretion, initiative, and independent judgement within established guidelines.

SALARY
Compensation is based on a five-step salary range, with annual merit-based increases within the salary range.

RETIREMENT

HOLIDAYS AND PERSONAL LEAVE
Mendocino County observes 11 paid holidays per year. Employees are granted 24 - 48 hours of personal leave annually, depending upon Bargaining Unit.

VACATION
Accrues at the rate of two weeks per year for three years. Three weeks per year after three years, four weeks after eight years and five weeks after 15 years.

SICK LEAVE
Paid sick leave accrues at the rate of 1.25 days per month, or 15 days per year. Accruals are pro-rated for part-time employees working at least 20 hours per week. Part-time employees working less than 20 hours per week, and extra-help employees receive up to 24 hours (or 3 days) of paid sick leave annually.

The County and the employee share the cost of an employee selected health care plan; enrollment in the plan includes $20,000 Life Insurance.
